Paper Presentation: Assessing Symptoms and Symptom Trajectories — Symptom Trajectories among Adolescents during Hematopoietic Stem Cell Transplantation (216-1)

1CNE  Basis of inquiry: Symptom management is major role of pediatric hematopoietic stem cell transplant (HSCT) nurses and a thorough understanding of symptom experiences is needed. Symptoms are complex and their frequency, severity, and distress change over time. Research is needed to identify symptoms over time (trajectories) among adolescents undergoing HSCT.

The aim of this study was to describe symptom trajectories including symptom frequency, severity, and distress among adolescents from pre-HSCT to 100 days post HSCT.

Methods:This study used a repeated measures design among 58 adolescents age 10–19 years. Researchers obtained demographic information from the medical record. Adolescents completed the Memorial Symptom Assessment Scale 10–18 pre-HSCT, then every 30 days for three additional time points (Day +30, Day +60, and Day +90). Descriptive statistics were used to summarize subject characteristics. Latent class growth analysis was used to classify patients into distinctive symptom groups. Logistic regression was used to identify any relationships of demographic and treatment characteristics on the latent classes of symptom trajectories.

Findings or Outcomes:
Two latent class trajectories were identified from the symptom reports. Symptom frequency trajectories included 68% of patients having low number of symptoms that were stable over time while 32% of patients had a higher number of symptoms that peaked at Day +30 post HSCT. Symptom severity trajectories consisted of 40% of patients having low symptom severity and 60% of patients having more symptom severity; both trajectories remained stable over time. Symptom distress trajectories included 65% of patients having low symptom distress and 35% of patients having more symptom distress; both trajectories remained stable over time. The relationship of demographic and treatment characteristics to the symptom trajectories will be presented.
